Application for an account and card of the FALA System (to download)

You can download the application for opening an account and obtaining a card in the FALA System here. The completed application should be submitted to your chosen customer service point. The card will be ready for pickup at the same location where you submitted the application. We will notify you via SMS once it's available for pickup.
