How to order your first FALA card:

• Via the portal (systemfala.pl):

  1. Log in at www.systemfala.pl.

  2. Go to the "Settings" tab, then select "Identifiers".

  3. Click "Add identifier".

  4. Select "FALA card" and follow the on-screen instructions.

• In person:

  • Visit any Customer Service Point (POK).

If you already have a FALA card and would like to order another one, you can do so at a Customer Service Point or via the website:

• Log in to systemfala.pl

• Go to the "Zgłoszenia" (Requests/Support Tickets) tab, then select "Utwórz nowy" (Create new).

• Choose the request category "Wniosek o wydanie identyfikatora" (Application for an identifier/card issuance).
